Can I share a main folder on syncthing and choose who gets access to the subfolders, without create them on syncthing?

Sorry, no. I think you should read Getting Started — Syncthing documentation , if you haven’t already.
The related issue is https://github.com/syncthing/syncthing/issues/7836. With per-folder ignore patterns, you could simply add the same path to Syncthing multiple times, and then share each of them with different ignore patterns. This isn’t possible at the moment, because ignore patterns are stored in a hard-coded .stignore file in the folder root.
